What to expect
- A confirmation call from our office to verify insurance and answer questions
- Your first visit in person at our Central Islip office (video follow-ups after)
- Intake paperwork sent through a secure portal before your visit
- An unhurried evaluation with no rushing and no interruptions
- A written plan and a scheduled follow-up before you leave
What to bring
- Photo ID and your insurance card
- A list of current medications, including doses
- Names and contact information for your other providers
- Any prior psychiatric records or testing you have access to

Fees

Cancellations
Please give at least 48 business hours notice. Late cancellations and missed appointments may be billed at the full visit rate.
Prescription refills
Request refills through your pharmacy at least five business days before you run out. Refills are provided in the context of a scheduled follow-up.
Records
Written requests for records are processed within 30 days as required by law.
Intake paperwork
