How this website handles the information you send us.

Information we collect on this website
When you submit the appointment request form we collect the details you enter: the patient's name, date of birth, and home address; insurance details (plan, member or Medicare number, and policyholder information if needed); a phone number and email address; any note you choose to add; how you heard about us; and, if you are requesting for someone else, your name and relationship to the patient. When you visit the site, our web host also keeps standard server logs, including IP address and browser type.
This form is not for emergencies
The appointment request form is not monitored for emergencies. If you are in crisis, call 911, or call or text 988 for the Suicide & Crisis Lifeline. Please keep any notes on the form brief; we cover your history privately at your visit.
How we use your information
We use what you submit only to respond to your request, check insurance coverage, and schedule an appointment. Your request is encrypted in transit and sent from your browser to a Microsoft Azure service, which delivers it to our office's Microsoft 365 email. Both services are covered by Microsoft's HIPAA Business Associate Agreement. Requests are not stored on this website's server or in a website database. We send an automatic confirmation email that does not repeat the details you submitted. We do not sell your information or use it for advertising.
Cookies and analytics
This website does not use advertising or analytics trackers, and it does not set tracking cookies. The one exception is the optional map on our Contact page. Nothing is loaded from Google unless you choose to show the map. If you do, Google may set cookies and receive your IP address and browser details, under Google's own privacy policy.
Notice of Privacy Practices (HIPAA)
Health information created during your care is protected under HIPAA and described in our Notice of Privacy Practices, which is provided at your first appointment. Your rights regarding that information are explained in that Notice, and you are welcome to call our office with questions about it.
