Self-pay rates are available and quoted before scheduling.
Cancellations
Please give at least 2 business days' notice to cancel or reschedule. Late cancellations and missed appointments may be charged the full visit fee.
Prescription refills
Request refills through your pharmacy at least five business days before you run out. Refills are provided in the context of a scheduled follow-up.
